HCB Case Management Waiver Services

 

Center for Accessible Living (CAL) is proud to provide HCB (Home and Community Based) Case Management Waiver services in Kentucky. Our goal is to help individuals access the supports and services they need to live independently and remain connected to their communities.

During the first months of implementation, CAL plans to provide services in the following Kentucky counties: Adair, Ballard, Calloway, Carlisle, Casey, Clinton, Cumberland, Fulton, Graves, Green, Hickman, Marshall, McCracken, McCreary, Metcalfe, Pulaski, Russell, Taylor, and Wayne.

 

After the initial phase, CAL plans to expand HCB Case Management Waiver services statewide to all counties across Kentucky.

Who May Be Eligible for HCB Waiver Services

The Home and Community Based (HCB) Waiver Program helps older adults and individuals with physical disabilities receive support services in their homes and communities instead of living in a nursing facility.

​

You may qualify for HCB Waiver services if you:

  • Are age 65 or older, or have a physical disability

  • Meet the level of care requirements for nursing facility services

  • Meet Kentucky Medicaid financial eligibility requirements

  • Need long-term supports to safely remain at home or in the community

How CAL Can Help

CAL’s case management team works alongside participants and families to help identify needs, connect with services, and support independent living goals through person-centered planning.

 

Our case managers may help participants:

  • Understand Medicaid waiver services and eligibility

  • Complete waiver applications and enrollment steps

  • Develop person-centered service plans

  • Coordinate medical, community, and support services

  • Connect with providers and community resources

  • Monitor health, safety, and changing support needs

  • Advocate for participant goals and independence

  • Access Participant Directed Services (PDS) options when available

Services and Supports Available Through the HCB Waiver

Services are based on each participant’s individual needs and approved plan of care. Available supports may include:

  • Case management

  • Attendant care and personal care assistance

  • Adult day health services

  • Home delivered meals

  • Respite care for caregivers

  • Environmental and minor home adaptations

  • Goods and supplies related to health and safety

  • Participant Directed Services (PDS) options

 

Examples of supports may include:

  • Assistance with bathing, dressing, and daily activities

  • Home accessibility modifications such as ramps or grab bars

  • Meal support and nutrition services

  • Caregiver relief services

  • Coordination with doctors, therapists, and other providers

  • Assistance locating community resources and benefits

Participant Directed Services (PDS)

Some participants may choose Participant Directed Services (PDS), which allows individuals greater choice and control over certain non-medical services. Depending on eligibility and program guidelines, participants may be able to hire trusted caregivers, including certain family members, friends, or neighbors.
