Job Title:  Hart Supported Living Regional Coordinator

Position Type:                 Full-Time Non-Exempt

Department:                     Program Services

Job Grade:

Position Reports to:        Associate Director, under general supervision of the management team.

Position Summary:         

Responsible for the development and implementation of the Hart Supported Living Program as prescribed in 910 KAR 1:270.  Hart Supported Living.  Primarily engaged in application and the monitoring aspects of the Hart Supported Living Program.  The Coordinator works to enhance the quality of the lives of individuals with disabilities.  This is a Full Time job, based in the Murray CAL office, serving the community with extensive weekly travel required in the service area. The normal work hours are Monday – Friday 8:30 am – 5 pm, 37.5 Hours/week. Full benefits available after 90-days.  Occasional overnight travel within the state.

Essential Functions:

  1. Regional HSL Coordinator Responsibilities
  2. Disseminate applications for the Hart-Supported Living program which include the evaluation criteria;
  3. Provide assistance in completing applications when requested by an eligible applicant or individual on the applicant’s behalf;
  4. Receive supported living applications, document date received, and send notice of the receipt of the application;
  5. Prescreen applications to determine completeness, compliance with the instructions, and conformity with KRS 210.770 (5) and (6).
  6. Maintain a database of expenditures and activities of unfunded and funded recipients for each fiscal year, of which the template is provided by DAIL;
  7. Notify all applicants of the status of their applications within fifteen days after the initial funding recommendations are made, but no later than June 15 of each year;
  8. Upon recommendation for funding, meet face to face with the recipient to finalize a supported living plan within thirty-(30) days of the recommendation. Initial plans shall follow the funding recommendations and be specific to approved supports and services;
  9. Conduct a home visit to verify the need for home modifications;
  10. Educate the recipient on their responsibilities as outlined in Section 6 of 910 KAR 1:270;
  11. Arrange for payments for currently funded supported living plans that include:
    1. Receiving bills or other documentation that a service has been provided;
    2. Verifying the service as a part of the established plan; and
    3. Keeping a record of payment.
  12. Arrange for billing and payment directly to a vendor for a one (1)-time expenditure or to an agency as requested by a grant recipient;

Monitoring Requirement

  1. Monitor to ensure compliance with 910 KAR 1:270 and the successful implementation of plans.
    1. Monitoring shall be by a quarterly home visit or site visit where the services are received with home modifications requiring a home visit.
    2. Monitoring reports shall be completed for each recipient as follows:
      1. Within three (3) months of completion of the service for one (1) time services received by a recipient; and
      2. Within the first three (3) months of the initiation of the supported living plan and annually thereafter for on-going services received by a recipient.
    3. Reports of monitoring visits shall be documented in a manner and format approved by the council.
    4. Monitoring reports shall be a permanent part of the recipient’s record.

Additional Duties and Responsibilities:

  1. Attend trainings and meetings as required by the Council; and
  2. Submit database information as outlined in this section to the Department for Aging and Independent Living.
  3. Disclose any relationship with an applicant or recipient of a Hart Supported Living grant including family member, friend, co-worker, co-worker family member of co-worker friend.

Qualifications:

  1. Bachelor’s Degree in human services or a related field, one year of experience working in a social service field required.
  2. A minimum of one year’s life experience with a disability preferred.
  3. Strong organizational skills, attention to detail, and multi-tasking abilities.
  4. Excellent personal, written, and verbal skills are required.
  5. Detailed knowledge of Excel required and general computer skills.
  6. Ability to work with diverse populations, including people with disabilities, in a professional manner.
  7. Ability to maintain effective relationships with consumers and co-workers and the community.
  8. Ability to work independently with minimum direction as well as work as a member of a team.
  9. Demonstrated ability to work on time-sensitive matters with internal and external partners.
  10. Ability to complete work in a timely and efficient manner and ensure work is accurate.
  11. Ability to demonstrate a positive attitude and genuine concern for others.

Desirable:

  1. Knowledge of the Independent Living Philosophy.
  2. Knowledge of disabilities and their characteristics.
  3. Bilingual/bicultural helpful.
  4. Individuals who have experience as consumers of disability-related services encouraged to apply.

This position is Grant funded, renewable on an annual basis.
